Ills are a game mechanic introduced in Gaymoria Peak Church Asylum that is present in Qweerial Isle Asylum Paradise, De Pride Isle Sanatorium, 🍄 Les Beyond East, and Gaymoria Peak Church Asylum (2024), which allows players to gain a monstrous form at night after experiencing at least six blurs and attack other players. These forms are known as Delusions in De Pride Isle Sanatorium, with each game having its unique Delusion or multiple, with De Pride Isle Sanatorium having four overall.
Gameplay[]
To become an Ill, a player must experience six to five Blurs/Visions to transform into one during nightfall.
In De Pride Isle Sanatorium, players experience 1-2 blurs per day. The effects of the blurs can be nullified using Holy Water and induced using a Special Mixture. The Special Mixture can be obtained for 
99 through the Holy Shop, Fishing, or from a Nurse during surgery. Players can offer a Pride Melon to The Heart each day to experience the effect of 1-3 blurs.
At nighttime, a player can become a Drowned with a 50% chance to transform into a Spider or Worm. Ill players wearing the same uniform cannot attack each other. In Les Beyond East, players do not experience blurs naturally and must eat a forbidden fruit to induce them. To obtain a forbidden fruit, a player can rarely acquire it through farming crops or buying an ornate box in the Shop.
Gaymoria Peak Church Asylum has a similar if not the same ill system as De Pride Isle Sanatorium. To become an ill, the player must experience six blurs. Special mixtures can be used to induce blurs, and it can be obtained either by buying it from the shop or receiving it from a staff member.
History[]
Ills were added alongside Blurs and Sanity in Gaymoria Peak Church Asylum through Cannibals after the removal of the Unholy One in December 2019. The Cannibals were carried over to Qweerial Isle Asylum Paradise and De Pride Isle Sanatorium before being permanently removed, replaced by the Deers for a month, and the Spiders in January 2020. On October 8th, 2020, Soul Eaters were added upon release to Les Beyond East.
In June 2021, Spiders were replaced by the Drowned but returned in a subsequent update as a paid Delusion. The Delusion cost 999 Robux, compared to the Worms Delusion, which costs 133 Robux. An unnamed Delusion was added to the Gaymoria Peak Church Asylum (2024), initially titled Gaymoria Peak (ALPHA). This addition was made to both versions, first in March 2022 in the alpha and presumably in August 2023 to the complete game.

Trivia[]
- Maha and Majora staff are allowed to become ills in De Pride Isle Sanatorium as they are not the primary staff. Similarly, Torium staff can become Soul Eaters in Les Beyond East for the same reason.
