De Pride Isle Sanatorium is an all-genre Roblox game created by Divine Sister. It is the spiritual successor to Qweerial Isle Asylum Paradise, of which it was initially a bombed retexture. Players are tasked with surviving in a sanatorium on a remote island, where they are cared for by nurses and, at night, must avoid or even become ill(s).

Map
Spawn
When initially spawning into the experience, the player awakens in a cave full of sand and rocks. After exiting the cave, you see a steep pathway leading upwards with an arrow sign saying, "Sanatorium." When you go up, you will see a zipline. You must own the golden ticket game pass (999 robux) or the temporary ticket (111 robux) to ride the zipline, a faster way to get to the sanatorium.
Beside the "Sanatorium" arrow sign is another pathway that leads onto the beach. Near the beach is a dock and another zipline. Jack, the gay pirate, and his boat stop there. Upon boarding, Jack will take you to the previous spawn island, where the cabins and camping ground are. This is where the camping trip activities are hosted.
Village De Pride
After going up the hill and continuing down the pathway, you will soon come across a second pathway. While going down the second pathway, you will come across another "Sanatorium" arrow sign. There will be an archway with the words "Plaza De Pride" on it. Going through the archway and down the pathway, you will find yourself in the village. If you turn right, you will see another archway with the words "Plaza De Pride" on it. Through the archway is a market with a stage, used for drama plays and other activities during a village trip.
There are 11 houses within Village De Pride, a cafeteria with a kitchen, two tables on the first floor, and six on the second floor. Next to the cafeteria is a wide area with seven picnic tables, a small stage, a fountain, and a few market stalls.
Going down the village, you will find another archway with the words "Plaza De Pride" on it. Through the archway is a bridge, a part of the old spawn island, which you would have to cross and go through the village to get to the sanatorium. Crossing the bridge now, only accessible from the village, leads to a large island. The island is primarily empty, besides two tents with picnic tables underneath.
Heading to the Sanatorium
Upon exiting the spawn location, you find a path that goes up. Go up the pathway and continue following it. Soon, you will reach a crossroads with another pathway. Stick to it on and keep going forward. You will eventually see a large set of stairs that is the sanatorium.
Gameplay
Players begin the game by awakening in a cave and venturing to the sanatorium. Inside, players must equip one of four patient uniforms in the Uniform Hall, being welcomed by nurses and served breakfast each morning. During the day, nurses conduct various activities between breakfast and the shipment. On Sundays, nurses will distribute Holy Water to the players after the shipment.
Players manage their character's stats, including Vitality (health), Energy, Hunger, and Hygiene. Energy naturally replenishes when resting. Vitality can decrease by falling from a high place or attacks by ill (insane) players. Showering in the bathing quarters maintains Hygiene. Every day, the player will experience blurs, increasing their insanity, which can be nullified with Holy Water and induced with a Special Mixture or an offering to The Heart.
Activities conclude at noon, followed by dinner. Nurses then instruct players to return to the dormitories, threatening isolation if caught roaming. A 60-second countdown initiates before the dormitories are locked. Players who have experienced six blurs may become a Drowned, Spider, or Worm (if they have the game pass, which costs 999 robux for the Spider and 111 robux for the Worm). Once transformed, they can attack and consume others until daytime when they will revert to normal, and dormitories will be unlocked.

- The original uniforms were ragged versions of Qweerial's. However, they were scrapped.
- In De Pride Isle's initial release, there was an abandoned home called the 1House (built by 1Crumb and Airashiyo in Qweerial Isle Asylum Paradise, who had since resigned). Eventually, the 1House was removed from the game to reduce lag.
- According to the trailer, the game is set in October 1985.
